Fidelis Medicaid Advantage Plus (MAP) is a new program for those with Medicare and Medicaid coverage who have a chronic illness or disability, and are eligible for nursing home care. Each member has his or her own Care Manager who coordinates services with the member, the member’s family, doctor, and other caregivers in the community.
