Stay Covered With Fidelis Care

Once each year, you must renew your coverage. It’s easier than you think!

Each year, Child Health Plus, Family Health Plus, and Medicaid Managed Care members in New York State must renew their health coverage. This process is called recertification. It may seem confusing, but it’s important to take the time to complete and return the forms that you receive. If you do not do so, your coverage will end.

Fidelis Care can help you keep your health coverage and walk with you every step of the way. Here is some helpful information about recertification and what you can expect:

If You Have Family Health Plus or Medicaid Managed Care Coverage
About 30 days before your coverage is set to end, your local Department of Social Services (LDSS) or the Human Resources Administration (HRA), if you live in New York City, will mail a recertification package to you. To keep your coverage:

  • Review the information carefully and make any necessary changes
  • Sign the forms
  • Return them by the date given

A Fidelis Care representative will contact you by phone to remind you to fill out and return the paperwork. The Department of Social Services or HRA will review your information and determine if you are still eligible for Family Health Plus or Medicaid Managed Care. If you are, Fidelis Care will continue as your health plan.

If You Have Child Health Plus
Ninety days (or 3 months) before your or your child’s coverage is set to end, Fidelis Care will mail you a recertification package. Fill out and return the information as soon as possible. If we do not receive information right away, we will mail you reminder letters and follow-up with a phone call. Once all of your information is received, we will review it to determine if your child's monthly premium has changed. Fidelis Care will continue as your health plan.
