Have you paid your monthly premium this month? Fidelis Care would like to remind Child Health Plus members and their parents or guardians that timely payments of monthly premiums is very important.

Based on New York State guidelines, if Fidelis Care does not receive the full premium by the last day of the month prior to the coverage month, your child's insurance will end on the last day of the month for which the premium was paid. It is a good idea to put your premium payment in the mail at least 10 days before it is due.
