Fidelis Medicaid Advantage Plus (HMO) is for individuals who have Medicare and Medicaid coverage and have a chronic illness or disability. You must require health and long-term care services and may be in need of nursing home level of care. Members receive individual attention and flexible care planning to help them live in their own homes and communities for as long as possible. Enrollment in the program includes coverage for all of your Medicare and most of your Medicaid services. As a member, you'll have access to a large network of doctors, hospitals, and community-based services across New York State.
